Before you fill out this form, read Information on Appeal Procedures for

The court is required to appoint a lawyer to represent you if you are indigent
(you cannot afford to pay for a lawyer) and:

paying a fine of more than $500 (including penalty and other
assessments); or
You are likely to suffer other negative consequences from the
conviction (for example, immigration problems or inability to get or
keep a license or permit); or
(3)
You have not been convicted but you are likely to suffer significant
harm if you lose the appeal.

Take or mail the completed form to the clerk’s office for the same trial court
where the notice of appeal was filed. It is a good idea to take or mail an extra
copy to the clerk and ask the clerk to stamp it to show that the original has

Notice to Defendant: If you were represented by appointed counsel in the trial court and the trial court
finds that you are able to pay all or part of the cost of that counsel, at the conclusion of the
proceedings, the court may also determine after a hearing whether you are able to pay all or a portion
of the cost of any attorney appointed to represent you in this appeal. If the court determines that you
are at that time able to pay, the court will order you to pay all or part of such cost. Such orders will
