Sex Offender Travel Notification Document
The Alabama Sex Offender Registration and Community Notification Act1 (“Act”) requires sex offenders to
complete and sign a travel notification document prior to travel. This document summarizes the responsibilities
of a sex offender when traveling away from his or her county of residence for three (3) or more consecutive days
and requires the offender to provide dates of travel, travel location and lodging information prior to travel.

Special Duties for Sex Offenders while Traveling:
• Adult sex offenders who leave their county of residence for travel inside the United States of America
for a period of three (3) or more consecutive days must report in person, to the sheriff in each county of
residence, and provide the above information within three (3) business days prior to travel. Upon
return from such travel, adult sex offenders must report to the sheriff within three (3) business days of
return in each county of residence.
• Adult and juvenile sex offenders traveling outside of the United States of America must report in
person, to the sheriff in each county of residence, and provide the above information at least 21 days
prior to leaving their county of residence. If the travel to another country is for a family or personal
medical emergency or a death in the family, the offender must report in person, to the sheriff in each
county of residence, within three (3) business days prior to travel. Upon return from such travel, sex
offenders must report to the sheriff within three (3) business days of return in each county of residence.
• Sex offenders must sign this form stating that they understand the requirements above.
• Any violation of any of the above provisions constitutes a Class C felony.

to comply with any provision of the Act, I understand that I may be charged with a Class C felony in Alabama. I
