The defendant is seeking a Certificate of Eligibility to Register to Vote through the Alabama Board of Pardons and Paroles.
The defendant was convicted of a felony or felonies in the above referenced case(s).  The felony or felonies of which the
defendant was convicted is/are marked on the second page of this form.
The felony or felonies of which the defendant was convicted is/are a crime(s) of moral turpitude pursuant to §17‐3‐30.1,
Ala. Code 1975.  The defendant has been disqualified from voting by reason of the conviction(s).
At the time of sentencing in these case(s), the defendant was ordered to pay court costs, fines, fees, and/or victim
restitution.
Section 15‐22‐36.1, Ala. Code 1975, requires a person seeking a Certificate of Eligibility to Register to Vote to pay all fines,
court costs, fees, and victim restitution ordered by the sentencing court at the time of sentencing on disqualifying cases,
among other things.
After the time of sentencing, the defendant has been ordered or otherwise directed to pay a collection fee or other fees
pursuant to state law OR has been ordered to pay court ordered funds in other criminal cases which are not disqualifying
felonies under § 15‐22‐36.1, Ala. Code 1975.  These additional sums were not ordered to be paid at the time of sentencing
but have been incurred after the time of sentencing.
In effort to obtain a Certificate of Eligibility to Register to Vote, the defendant will arrange to pay court costs, fines, fees,
and/or victim restitution that were ordered at the time of sentencing in the case(s).

The defendant requests that the court enter an order directing the clerk to reprioritize the court costs, fines, fees, restitution, and 
other court ordered monies ordered to be paid in this case so that that any money paid in this matter by the defendant will first be 
applied to all fines, court costs, fees, and victim restitution ordered by the sentencing court at the time of sentencing in disqualifying 
cases.

Pursuant to §15‐22‐36.1(g) “A person who has lost his or her right to vote by reason of conviction in a state or federal court for any of the following will not 
be eligible to apply for a Certificate of Eligibility to Register to Vote under this section: Impeachment, murder, rape in any degree, sodomy in any degree, 
sexual abuse in any degree, incest, sexual torture, enticing a child to enter a vehicle for immoral purposes, soliciting a child by computer, production of 
obscene  matter  involving  a  minor,  production  of  obscene  matter,  parents  or  guardians  permitting  children  to  engage  in  obscene  matter,  possession  of 
obscene matter, possession with intent to distribute child pornography, or treason.”

ORDER OF THE COURT 

Upon consideration of Request by the Defendant, the Court orders the following to facilitate compliance with Alabama Code §15‐22‐36.1.  
The clerk is hereby directed to reprioritize the court ordered monies in these cases.  All funds paid in these cases shall be applied in the 
following manner:

Future payments in these cases will first be applied first to court costs, fines, fees, and victim restitution ordered to be paid on the
date of sentencing in the disqualifying offenses (as described in §15‐22‐36.1) listed as follows:
