information is necessary to ensure full and proper execution of this Court’s Order:

[ ] The Charge or Conviction to be expunged.
[ ] The date of arrest for the Charge or Conviction to be expunged.
[ ] The name and address of the agency or department that made the arrest.
[ ] The Court where the Charge or Conviction was disposed.
[ ] Other:
The Petitioner is granted sixty (60) days from the date of this Order in which to correct each of the
errors or omissions specified above. After sixty (60) days, the Court will reconsider the case file and issue
an Amended Order. If the errors or omissions have not been corrected, the Court’s prior Order will be
VACATED.
The Clerk is directed to provide a copy of this Order to the Petitioner’s attorney or to the Petitioner
at the address on record if the Petition is filed pro se.
