WHEREFORE, the premises considered. The Plaintiff petitions this Honorable Court as follows: 

1.  Cause a copy of this Petition and a Notice/Summons to be served upon the defendant as provided in § § 30-3-62 and 25-4-152, 

Code of Alabama 1975, of the proposed Withholding Order in this cause and the Court’s duty to enter said Order unless said  

Defendant requests a hearing within the time provided therein.  

2.  Enter an Order as required by § § 30-3-62 and 25-4-152, Code of Alabama 1975, directing any employer of the Defendant or  

The Department of Industrial Relations to withhold and pay out of income/benefits due or to become due the obligor at each 

pay period, an amount the Court finds necessary to comply with the original Order of Support plus an amount reasonably 

sufficient to satisfy any accumulated arrearage.
