Standard License or ID Temporary Permit for Adult Sex Offenders 

The Alabama Sex Offender Registration and Community Notification Act1 (“Act”) requires adult sex offenders to 
obtain a valid driver license or identification card issued by the Alabama Law Enforcement Agency (“ALEA”) with 
a designation that enables law enforcement to identify the licensee as a Sex Offender within fourteen (14) days 
of registration. This form allows an adult sex offender to carry his or her non-designated license or identification 
card for thirty (30) days from the Date of Issuance listed below if the offender’s information is not yet entered 
into the State Sex Offender Registry. 

At the expiration of this time period, I understand I am required to return to the driver license office to obtain a 
driver license or identification card with the sex offender designation placed on it. At this time, I understand that 
I am to relinquish, to ALEA, any other driver license(s) or identification card(s) previously issued to me by a state 
motor vehicle agency which do not bear any designation enabling law enforcement officers to identify me as a 
sex offender. 

By signing below, I acknowledge that I have read the above information and responsibilities and that I am aware 
of all that is required of me under the Alabama Sex Offender Registration and Community Notification Act. If I fail
