NOTICE TO DEFENDANT, DEFENDANT’S EMPLOYER AND DEPARTMENT OF INDUSTRIAL RELATIONS
he defendant, the defendant’s employer and the Department of Industrial Relations are hereby notified that
pursuant to §30-3-62(h), Code of Alabama 1975, this order for termination of withholding upon payment of the
arrearages is available to the defendant only once. Defendant is advised that this order does not terminate the
original order of support. You are still obligated under the original order of support to continue making your
support payments. If an arrearage reoccurs pursuant to the same support order, a withholding order must again be
issued and shall be permitted for the duration of the obligation for support.
