Property which is used or intended for use as a container for property described above; and/or
Conveyances including aircraft, vehicles, or vessels, or agricultural machinery, which are used, or are intended for use, to
transport, or in any manner to facilitate the transportation, sale, receipt, possession, or concealment of any property described
above; and/or
Books, records, and research products and materials, including formulas, microfilm, tapes, and data, which are used or
intended for use in violation of any law of this state concerning controlled substances; and/or
An imitation controlled substance, as defined under the laws of this state; and/or
Real property or fixtures used or intended to be used for the manufacture, cultivation, growth, receipt, storage,
handling, distribution, or sale of a controlled substance in violation of any law of this state; and/or
Property of any type whatsoever instituting, or derived from, any proceeds obtained directly, or indirectly, from any
violation of any law of this state concerning controlled substances.
This property is therefore evidence of a crime and subject to forfeiture pursuant to Section 20-2-93, Ala. Code,1975. After obtaining
the Seizure Order, I will present it, along with an application for forfeiture, which shall include an inventory, to the
________________ County District Attorney’s Office, within 14 days after obtaining the Seizure Order, which may file a
complaint for a forfeiture action regarding the property in Civil Division of the Circuit Court, in accordance with law.

Upon a finding of probable cause by the District Attorney that this seizure is valid, the District Attorney may file a forfeiture action
within 42 days from the date of the seizure of the property (or longer time upon motion and order of this court for good cause
shown) pursuant to Sections 20-2-93(f) and (g), Ala. Code 1975.
Should the District Attorney not file a forfeiture action pursuant to Sections 20-2-93(e)(3) and (f), Ala. Code 1975, after 90 days
from the date of this Order, if the undersigned judge dismisses the case due to inaction, the property shall be made available by
the seizing agency for return to the owner, upon documented proof of ownership, within 14 days after the dismissal EXCEPT
(1) upon motion by the District Attorney pursuant to Section 20-2-93(e)(4), Ala. Code 1975, to retain the items as evidence in the
underlying investigation or case, OR (2) if the property is contraband in which case it shall be destroyed at the conclusion of
the criminal case, should it not be needed or used as evidence pursuant to Sections 20-2-93(e)(3) and/or (f), Ala. Code 1975.
